List of Acceptable HIV Status Documentation
Please bring with you any one (or more) of the items listed below to the enrollment visit:
If you’re currently taking Truvada for PrEP you need to bring:
- If HIV negative, a copy of your most recent HIV negative test result clearly labeled with your name or other identifier and date of HIV test (result must be within the last 6 months).
- If HIV positive a copy of your test result or other written confirmation such as an M11Q, lab results or a patient print-out summary obtained from your medical or other health care provider.
- If HIV positive a written prescription for your HIV medication clearly labeled with your name, date and signed by the prescribing authority, e.g. MD, PA, etc.
- If HIV positive your HIV medication bottle clearly labeled with your name, date and prescribing authority, etc., (can also substitute a photo of the prescription bottle).
If you’re currently taking Truvada for PrEP you need to bring:
- Your Truvada medication bottle clearly labeled with your name and prescribing authority, date, etc., (you can also substitute a photo of the prescription bottle).
- Or a written prescription for your Truvada medication clearly labeled with your name, date and signed by the prescribing authority, e.g. MD, PA, etc.
